This course will address challenging ERP cases. We will tackle obstacles like motivation, TIBs, and other ERP challenges, and also consider dual diagnoses such as PTSD, Autism, and ADHD.

CLASS DESCRIPTION
This course will explore complex and challenging ERP (Exposure and Response Prevention) cases across a range of diagnoses, including Obsessive-Compulsive Disorder (OCD), Phobias, Generalized Anxiety Disorder (GAD), and Panic Disorder. Clinicians will learn to identify and navigate common barriers that interfere with effective ERP implementation.
We will focus on how to recognize and address key obstacles such as:
- Motivation or ambivalence
- High symptom intensity
- Poor insight or inconsistent buy-in
- Ineffective exposure selection or culturally inappropriate exposures
- Treatment-interfering behaviors (TIBs)
- Family accommodation
- Therapist avoidance or hesitation
Through a problem-solving approach, clinicians will gain practical strategies to manage each barrier, ensuring greater confidence and competence in session.
Additionally, the course will examine how trauma can amplify symptom intensity and complicate ERP. We will address the intersection of trauma with anxiety-based diagnoses and offer clinical tools for more effective ERP, trauma-informed treatment.
Finally, we will briefly explore dual diagnoses such as Autism and ADHD, with a focus on how these presentations can create unique challenges during ERP. For a deeper exploration of neurodivergence and ERP, participants are encouraged to take our companion course, *Advanced Neurodivergence: OCD*. -
CLASS OBJECTIVES
Participants will:
1. Identify complex ERP cases across diagnoses including OCD, Phobias, GAD, and Panic Disorder.
2. Recognize common barriers to effective ERP, including motivational issues, high symptom intensity, poor insight, treatment-interfering behaviors (TIBs), use of ineffective or culturally inappropriate exposures, family accommodation, and therapist avoidance.
3. Develop clinical strategies to problem-solve and address ERP barriers directly within sessions.
4. Explore how trauma can impact symptom intensity and interfere with ERP implementation and problem solve this barrier using a trauma-informed lens to ERP.
5. Briefly describe how dual diagnoses such as Autism and ADHD can influence ERP and identify appropriate adaptations.

KD Holmes, MS, LPC, EMDR CERTIFIED
KD is a practicing therapist over the past 16 years at KDH Counseling. She has worked extensively with Generalized Anxiety Disorder, Social Phobia, Specific Phobias, Obsessive Compulsive Disorder, and Post Traumatic Stress Disorder. She is Eye Movement Desensitization and Reprocessing Certified, and BTTI trained for treatment of OCD. She enjoys using biology as a foundation to guide behavioral approaches to treating mental health issues. She loves DBT, RODBT, and ERP because they address mental health treatment through tangible behavioral steps. Followed by trauma work that can be done through EMDR, and Deep Brain Reorienting to help treat trauma responses that may be contributing to the presenting problems. We never stop learning and this insatiable quest for learning continues to drive her personally and professionally.
